Confidentiality


Confidentiality is fundamental to the running of this service.

A set of clinic notes would have been made for you with a unique clinic number. You can be very confident that any personal information given to us will be carefully protected. At the very least, we need a date of birth and a postcode, which we will match up with your own personal clinic number. These notes remain within the clinic and there are no links to the general medical record system in the rest of the hospital.

There are occasions when full name and address are needed i.e. when patients are referred to services outside of this department, principally radiology (x-ray and ultrasound scanning).

Confidentiality is only breached in very exceptional circumstances and never without prior discussion with you the patient.
